CrossWalk is sponsored by the Baptist Churches of New England and held on the campus of Nichols College, Dudley, MA, July 15-20, 2024. I understand and certify that my/ my child’s participation in CrossWalk and its activities are completely voluntary. I recognize that certain hazards and dangers are inherent during CrossWalk activities and program which may include, but are not limited to, water activities, field games, volleyball, basketball, wall climbing and ropes course. I acknowledge that although CrossWalk and Nichols College have taken safety measures to minimize risk, CrossWalk and Nichols College cannot guarantee that the participants, equipment, premises, and/or activities will be free of hazards, accidents and / or injuries. I further recognize and have instructed myself/ my child in the importance of knowing and abiding by the CrossWalk and Nichols College rules, regulations and procedures for the safety of conference participants. In an emergency, I hereby give permission to the physician or hospital selected by the camp director to hospitalize, secure the proper diagnostic, laboratory and radiological procedures, and to order any necessary medications, injections, anesthesia, intravenous therapy, or surgery for myself/ my child named above. I understand that upon such an emergency attempts to contact me/ emergency contacts will be made immediately.
By signing, I also grant permission for the use of any video/photos taken for CrossWalk promotional materials.
Therefore, on behalf of myself and my minor child, I release Baptist Churches of New England, Inc., the host church (listed above), Nichols College, and their respective officers, directors, trustees, clergy, employees, members, agents, volunteers, independent contractors, and insurers from all liability for harm to myself/ my minor child for injuries or damage arising from or connected with my/ my child’s participation in the CrossWalk conference sponsored by Baptist Churches of New England, Inc.
