YEC is sponsored by the Baptist Convention of New England and will be held Janiary 31 - February 2, 2020 at the Sturbridge Host Hotel, Sturbridge MA. I understand and certify that my child’s participation in YEC and its activities are completely voluntary. I certify that my teen is physically able to engage and participate in the activities planned. I acknowledge that although YEC has taken safety measures to minimize risk, it cannot guarantee that the participants, equipment, premises, and/or activities will be free of hazards, accidents and / or injuries. I further recognize and have instructed my child in the importance of knowing and abiding by the YEC guidelines, regulations and procedures for the safety of YEC participants. I recognize that the hotel has a pool that will be available for use by students during set free times. In an emergency, I hereby give permission to the physician or hospital selected by the leadership to hospitalize, secure the proper diagnostic, laboratory and radiological procedures, and to order any necessary medications, injections, anesthesia, intravenous therapy, or surgery for my child named above. I understand that upon such an emergency attempts to contact me will be made immediately. By signing I also grant permission for the use of any video/photos taken for YEC/BCNE promotional materials.
Therefore, on behalf of myself and my minor child, I release the Baptist Convention of New England, Inc., the host church (listed below), Sturbridge Host Hotel, and their respective officers, directors, employees, agents, volunteers, independent contractors, and insurers from all liability in tort for harm to my minor child for injuries or damage arising from or connected with my child’s participation in the YEC program sponsored by Baptist Convention of New England, Inc.
